Step-by-Step Guide: Entering the Australian Medical System as an IMG

Becoming a doctor in Australia as an international medical graduate requires navigating multiple regulatory steps. Here's a practical roadmap to get you started on your Australian medical career.


Step 1: Verify Your Credentials and Eligibility


Begin by using the AHPRA Self-Assessment Tool to determine which registration pathway suits your qualifications. Next, initiate primary source verification through ECFMG's MyIntealth platform—this mandatory step involves ECFMG contacting your medical school directly to verify your degree. Registration as valid in ECFMG's system typically takes 4-8 weeks. Ensure you meet English language requirements (IELTS 7.0 in Acedamic or equivalent). This step costs approximately USD $300-500.


Step 2: Choose Your Registration Pathway


IMGs have three primary pathways to registration. The Standard Pathway requires passing the AMC MCQ examination (multiple-choice questions testing medical knowledge) followed by either the AMC Clinical Examination or Workplace-Based Assessment (WBA). The Competent Authority Pathway applies to doctors from recognised competent authorities (UK, Ireland, Canada, USA, NZ) with recognised qualifications and practising experience. The Specialist Pathway is for specialist-qualified IMGs seeking specialist registration. Most IMGs pursue the Standard Pathway.


Step 3: Pass the AMC MCQ Examination


Create an account on the AMC Candidate Portal and register for the AMC MCQ exam. This computer-adaptive test assesses medical knowledge across clinical domains. The exam costs approximately AUD $2,000-2,500. Preparation typically takes 3-6 months depending on your background. Upon passing, you receive your AMC MCQ Certificate, advancing you toward Limited Registration.


Step 4: Obtain Limited Registration and Secure Employment


Apply to AHPRA for Limited Registration with your AMC MCQ Certificate. AHPRA now processes applications within approximately 7 days. With Limited Registration, you can work in Australian hospitals under supervision. Simultaneously, secure a hospital position—most IMGs work in public hospitals through state health services. Some positions explicitly support IMGs seeking WBA; these are advertised as "WBA-accredited" placements. Alternatively, complete the AMC Clinical Examination to progress toward Provisional Registration, though this requires expensive international travel and exam fees (typically AUD $4,000+).


Step 5: Complete Clinical Assessment—Two Options


Option A: Workplace-Based Assessment (WBA). With a secured hospital position, complete the WBA program (6-12 months). You're assessed continuously using tools including Mini-CEX (Mini Clinical Evaluation Exercises), DOPS (Direct Observation of Procedural Skills), and case-based discussions. You earn salary while being assessed in real clinical settings. Over 20 WBA-accredited hospitals operate nationally. Successful completion leads directly to Provisional Registration.


Option B: AMC Clinical Examination. Sit the AMC clinical exam internationally to obtain Provisional Registration without WBA. This requires international travel, costs approximately AUD $4,000+, and takes 3-18 months to complete depending on exam availability.


Step 6: Progress to General Registration


After 12 months supervised practice on Provisional Registration (or following Competent Authority Pathway approval), apply for General Registration. This step typically takes 4-6 weeks. General Registration permits independent clinical practice without supervision.


Timeline Summary


The complete pathway from initial application to General Registration typically requires 18-30 months. AMC MCQ preparation and examination: 3-8 months. Limited Registration approval: 1 week. WBA or clinical exam: 6-18 months. General Registration: 4-6 weeks.

Success depends on early credential verification, strategic pathway selection, and securing appropriate hospital sponsorship early. Begin immediately with ECFMG verification—this is your essential foundation.
